Billing, Fees, and Insurance

Self-Pay Service Fees:

Initial Intake Session: $2575 | Ongoing 60-Minute Sessions: $200 | Diagnostic Assessment: $500* | SSP Add-On: $15/Session | Document Rendering: $15-$45**

I offer a limited number of sliding-scale slots for individuals in need. Please reach out for further details or to inquire about open slots.

No-Show / Late Cancellation Fee:

Cancellation or rescheduling appointments requires 24 hours' notice. Late cancellations or requests to reschedule (made less than 24 hours prior to the scheduled session) will be subject to a fee ($50). No-shows*** will also be subject to a fee ($75).

Payment Policy:

 All balances/fees (e.g., session fees, copays, late cancellation/no-show fees) must be paid before the next scheduled service, or the appointment may be canceled. 

Insurances**** Accepted (PENDING):

*At this time, diagnostic assessments are not billed through insurance and are subject to the full self-pay fee.

**Documents include: Academic / Occupational Accommodation Letters, Leave of Absence (LOA) / Family and Medical Leave Act (FMLA) Letters, Emotional Support Animal (ESA) Letters, and Disability Documentation Letters. Please see the Informed Consent for additional information regarding documentation requests. Documentation fees cover the time required to review the client's record, complete requested documentation, and prepare the completed document. Payment of a documentation fee does not guarantee that a requested document will be completed or that the clinician will make a particular recommendation. 

***Appointments are considered 'no-shows' if the client is not present within 15 minutes of the scheduled session start time.

****At this time, I am not accepting insurance. All services rendered will be subject to the self-pay fees listed above unless otherwise agreed upon. Please contact the practice prior to scheduling to confirm that your specific insurance plan is accepted. While the practice will make reasonable efforts to verify insurance eligibility and benefits prior to the first appointment, verification does not guarantee coverage or payment. You are responsible for confirming your plan's covered services, applicable copays, coinsurance, deductibles, cost-sharing requirements, and any other applicable benefit limitations or exclusions. You remain responsible for any balance not covered or paid by your insurance plan.
